Fees
Fees for therapy:
$175 for Assessment and Diagnosis, 60 minutes
$160 for Individual, 60 minutes
$140 for Individual, 50 minutes
$165 for Family, 60 minutes
$170 for Couples Counseling, 60 minutes
$150 for Parenting Consultation, 60 minutes
Self-Pay
Payment is due at the time of each session. For cash pay, I take cash, check, debit, visa, Venmo, and PayPal. I take First Choice Health insurance and am out-of-network with Kaiser Permanente. For all other insurance companies, I can provide a Superbill which you can submit to your insurance company for reimbursement as an out-of-network provider.
Good Faith Estimates
Under the No Surprises Act, healthcare providers are required to give clients who don’t have insurance or who are not using insurance an estimate of the bill for services. The Good Faith Estimate show the cost of services that are reasonably expected for your healthcare and treatment needs. This will be provided as a part of your intake documents or upon your request.

Intake & Forms
There are two options available for completing my intake and forms prior to your first appointment.
Option 1: Go paperless and save a tree!
Provide your email and I will provide a link to my patient portal to complete the intake forms and sign online. You will have the option to add a credit card as well as your insurance information. Takes about 5-20 minutes to complete. Easy peasy.
Option 2: Tried and true!
Download, complete, and print either the Adult Intake Packet or Child/Adolescent Intake Packet and bring to your first appointment. These “packets” include both the Disclosure & Informed Consent and Confidentiality/Notice of Privacy Practices (HIPAA).

Cancellation Policy
You must provide 24 hours’ notice when canceling or making changes to existing appointments. Otherwise, you will be charged the full amount of the session you missed. Of exception, are emergencies and unexpected illness and/or known exposure to COVID-19. If you are using insurance, they will not pay for a missed appointment. You are responsible for the full fee. To cancel an appointment, please contact me at 360-888-7524. Please leave a message stating your name and that you are canceling your appointment.
